Under general supervision from the Director of Nursing, the Referral Coordinator will serve as a single point of contact for our referring providers, patients and caregivers to facilitate care coordination services for FHS patients.  The Referral Coordinator is expected to exercise a high degree of initiative and judgment in coordinating patient care and follow-up.  The Referral Coordinator will work closely with the clinical staff and patients to ensure referrals are made, kept, tracked and reported on.


  • High school diploma or GED equivalent
  • Excellent communication skills
  • Possess good people skills to work with patients, clinical staff and specialists
  • Bilingual English and Spanish required
  • Medical terminology experience preferred
  • Proficient with Microsoft Office Programs
  • Knowledge of local community resources preferred
  • Possess good organizational and time management skills
  • Must be able to exercise discretion and patient privacy
  • Ability to work independently and work collaboratively as member of a team
  • Valid Idaho Driver's License
  • Ability to pass a background check

Each employee will be trained on the FHS Medicare Compliance Plan and the Standards of Conduct and asked to sign a Conflict of Interest Statement at hire and annually thereafter.  A copy of the Medicare Compliance Plan and the FHS Standards of Conduct are available on the FHS home page under the Compliance tab.

Employee must read and understand the general and specific operational, safety, and environmental requirements of all plans, procedures, and policies pertaining to this job. 


Most work is performed in a typical interior/office work environment. Some will be performed outside of the office in a variety of settings including other agency offices, client homes, schools, etc. where terrain and accessibility are unknown.  Must be able to carry forms and supplies up to 15 lbs.
